A final pay and PTO record sheet writes down what to confirm about your last paycheck — timing, unused paid time off, outstanding expenses — as questions for HR, with room for their answers. It's a record you fill in, deliberately not a calculator.
When to use it
- Before your last day, while asking is still easy.
- Your exit has moving parts — unused PTO, a pending reimbursement, a bonus date.
- You want the answers written down rather than remembered.
Getting it right
- Ask "when" as much as "how much" — timing questions cause most of the surprises.
- Bring pay stubs to the conversation; concrete numbers get concrete answers.
- Write HR's answers onto the sheet during the conversation, with the date and who said it.
It does not determine what you are owed — payout rules for final wages and unused PTO differ by state and by policy, which is exactly why this sheet collects questions and answers instead of computing amounts.
